This will allow you to have a physical copy of the readings for review by yourself and your veterinarian.

Use this form to manually record your pet's daily glucose readings, including date, time, and notes. Print these blank forms as needed from the website or you can download them to your device for future use. After filling out the form, you can enter them into the Pet Diabetes Tracker on our website. Taking notes as to when your pet has eaten or when they were given medication or given an insulin shot will help you and your veterinarian better understand the glucose levels your pet is experiencing

Use this form to manually track glucose levels throughout the day at regular intervals as a "glucose curve" to better understand your pet's diabetes patterns.
Tracking your pet's glucose levels during regular intervals (every hour, 2 hours, 3 hours, etc.) and taking notes as to when your pet has eaten or when they were given medication or given an insulin shot will help you and your veterinarian better understand the glucose levels your pet is experiencing at different times of the day and why the levels may be listed at where they are.
